Yeah it's definitely by X Factor. Noticed that a while ago when I was messing around with some players. Give NTL everything and he'll jump in the top 3.

 Yes, change your activated players.  Your "Top 3" are chosen like this. 1) Look at activated players.  Pick the top 3 OVRs (not sure how ties are broken). 2) If #1 did not contain 3 players, pick the next X highest Overalls, where X is 3 less the number of players chosen in #1.

I think this is the case as well.  I have 99's in salary cap but not in regular since I don't have any abilities activated in salary cap or on my 99's (Minkah and Troy)
